- Coordinating clinical requirements, including client reviews, skill mix, and competencies for our care teams.
- Developing individual care plans in collaboration with clients, families, and healthcare professionals.
- Maintaining key relationships with stakeholders, including clients, families, and relevant agencies.
- Delivering care services aligned with our organisation's policies and procedures.
- Undertaking administrative tasks associated with the role.
We are seeking a Registered Health Professional with a strong background in healthcare, who is passionate about delivering high-quality care services. You will have:
- Current annual practicing certificate with the relevant regulatory body.
- Current driver's license (Full Licence Class 1).
- Knowledge of working with older people and/or those with disabilities in the community.
- Excellent interpersonal skills, team building skills, analytical and problem-solving skills.
- Competent in working with human services databases and Microsoft Word.
We would love to hear from candidates with experience in:
- Working knowledge of the Health & Disability Sector Standards and robust quality systems.
- Understanding of the social model of service delivery in a community care environment.
