Job Title:

High-Quality Care Specialist

HammondCare at Home is seeking a skilled High-Quality Care Specialist to provide personalized clinical care to clients in their own homes.

Job Responsibilities

The successful candidate will work collaboratively with the care management team, providing guidance to care staff and ensuring the delivery of high-quality clinical care. Key responsibilities include assessing client needs, developing care plans, and collaborating with clients and stakeholders to achieve optimal outcomes.

Required Skills and Qualifications
  • Nursing qualification and current registration with AHPRA as a Registered Nurse (Division 1, General).
  • Passion for and previous experience in client-focused service delivery in aged and community/home care settings.
  • Knowledge of Aged and Home Care Reforms and Aged Care Quality Standards.
  • Excellent communication and collaboration skills.
  • A commitment to advocacy and person-centred care.
