Job Description

We are seeking a skilled Clinical Care Professional to join our team. As a key member of our healthcare team, you will be responsible for providing high-quality clinical care and support services to our participants in the Home Care Package space.

You will work closely with our team to ensure that our participants receive the best possible care, and that their needs are met in a timely and efficient manner.

Key Responsibilities:
  • Provide clinical care and support services to participants in their homes
  • Work collaboratively with other healthcare professionals to develop and implement individualized care plans
  • Maintain accurate records of participant interactions and care provided
  • Participate in quality improvement initiatives to enhance the delivery of care
Required Skills and Qualifications

To be successful in this role, you will need to possess:

  • A Registered Nurse qualification and current registration under the Australian Health Practitioner Regulation Agency (AHPRA)
  • Current unrestricted drivers' licence
  • NDIS Worker screening clearance
  • CPR Certificate (ongoing requirement for annual certification)
  • National Police Check
  • Australian work rights
