Care Coordinator Role

We are seeking a highly skilled and compassionate Care Coordinator to join our Transitional Care Ward team at Figtree Private Hospital.
As a Care Coordinator, you will play a vital role in assessing, planning, implementing, and evaluating nursing care to ensure that patients receive holistic support meeting their physical, emotional, and psychosocial needs.
Key Responsibilities- Conduct thorough patient assessments to identify individualized care needs
- Develop and implement personalized care plans in collaboration with multidisciplinary teams
- Monitor and evaluate patient progress, making adjustments to care plans as needed
- Foster strong relationships with patients, families, and healthcare professionals to promote seamless care coordination
- Diploma or degree in Nursing or related field
- Current registration with Australian Health Practitioner Regulation Agency (AHPRA)
- Proven experience in care coordination or similar roles
- Excellent communication and interpersonal skills
- Able to work effectively in a fast-paced environment
