Home Health Care Professional

We are seeking a skilled and compassionate Home Health Care Professional to provide medical care to individuals in their homes and other settings.

About the Role:

  • Deliver high-quality patient care, including diabetes management, dialysis, IV medications, wound care, and more.
  • Collaborate with patients' families and caregivers to ensure comprehensive care.
  • Develop and implement individualized care plans that meet the unique needs of each patient.

Key Responsibilities:

  • Provide medical care, including administration of medications and treatments.
  • Assess patients' physical and emotional needs, and develop plans to address them.
  • Collaborate with healthcare teams to deliver coordinated care.
  • Document patient progress and communicate with healthcare professionals.

Requirements:

  • Licensure as a Registered Nurse in the province of practice.
  • Experience in community health or home care is an asset.
  • A valid driver's license and access to a reliable vehicle are required.
  • A current Criminal Records Check and Vulnerable Sector Search may be required.
  • Ability to work independently and as part of a team.

Working Conditions and Physical Demands: This role requires a detail-oriented approach in a dynamic environment, with physical activity including lifting, carrying, bending, reaching, kneeling, and other movements that emphasize good body mechanics. Individuals in this role are required to walk, sit, stand, and climb stairs throughout the day, with some tasks requiring fine hand movements.
