As a Care Coordinator, you will work closely with our team to ensure comprehensive client-centred management plans are achieved for our clients. This involves conducting initial phone assessments, care planning, and goal setting, as well as coordinating in-home services to support clients receiving care at home.
You will build and maintain effective relationships with various stakeholders, monitor patient progress, and update care plans as necessary. You will also liaise with healthcare professionals and specialists, provide telephone support, education, and advice, and discharge planning including referrals to long-term services.
The ideal candidate will be an experienced clinician with a customer-focused approach and a drive to deliver best practice outcomes across our programs. You will bring a clinical background in an acute hospital setting and have 3-5 years' experience as a Registered Nurse with unrestricted registration.
