Career Opportunity: Aged Care Specialist

We are seeking a skilled and dedicated individual to fill the role of Aged Care Specialist. As a member of our team, you will play a vital part in delivering high-quality healthcare services to our community.

The Single Aged Care Assessment System has been implemented to simplify and improve access to aged care services. This includes assessments for all aged care services, including Aged Care Assessments and Residential Aged Care Funding Assessments.

Key Responsibilities:

  • Conduct thorough client assessments, reviews, and develop support plans to help clients achieve their goals and maintain independence in their own homes and community.
  • Participate in clinical triage and case conferencing to ensure effective collaboration and risk management.
  • Identify, monitor, and manage risks, escalating issues as necessary to maintain a safe and supportive environment.

To be successful in this role, you will possess demonstrated experience working in Australia or overseas, directly delivering services in aged care settings and/or to aged persons.

Required Skills and Qualifications:

  • Tertiary qualification in a health-related discipline directly related to health, aged care, or a specialist area such as Allied Health or Social Work.
  • Current unrestricted registration with the Australian Health Practitioners Regulation Agency or other relevant professional association.
  • Enrolled Nurses with at least one year's Aged Care Needs Assessment experience are encouraged to apply.
