The role of the Disability Pathway Navigator Health Manager Level 2 is pivotal within the South Eastern Sydney Local Health District. This position entails providing high-level specialised/technical support for the management of the National Disability Insurance Scheme Implementation and Disability Inclusion Action Plan.
Key responsibilities include providing support to the Disability Strategy Manager in developing processes, pathways, and training required to assist SESLHD staff in supporting people with a disability, their families, and carers in their journey through the healthcare system to accessing appropriate disability care.
Furthermore, the successful candidate will be responsible for facilitating clinical and systemic escalations and identifying and developing pathways for people with complex needs to access the NDIS or alternative supports.
This role requires the ability to develop and maintain effective working relationships with senior management and stakeholders, as well as experience in planning and evaluation at strategic and service levels.
