**Blood Cancer Supportive Care Navigator Role Summary**

Job Description:

The position of Blood Cancer Supportive Care Navigator plays a vital role in our organization's mission to deliver impactful services to clients and healthcare professionals. As part of our triage team, you will be responsible for receiving and managing all inbound referrals, phone calls, and online chat inquiries.

You will conduct thorough risk assessments and supportive care needs assessments for clients, providing them with accurate information regarding blood cancer and available supportive care services. Additionally, you will triage, coordinate, and facilitate access to these services across our service offerings.

Key Responsibilities:
  • Manage incoming referrals, phone calls, and online chat inquiries from clients and healthcare professionals.
  • Conduct comprehensive risk assessments and supportive care needs assessments for clients.
  • Provide accurate information on blood cancer and supportive care services.
  • Triage, coordinate, and facilitate access to supportive care services.
Requirements:

To succeed in this role, you will need:

  • A tertiary qualification in social work, nursing, or an allied health profession.
  • Registration or membership with the Australian Health Practitioner Regulation Agency (AHPRA) or respective professional body.
  • A comprehensive understanding of blood cancer and its impact on clients.
