Key Responsibilities:

  • Manage inbound referrals, phone calls, and online chats from clients and healthcare professionals regarding supportive care services.
  • Document and track all referrals and communications in relevant systems.
  • Conduct thorough risk assessments and supportive care needs assessments for clients.
  • Identify and prioritize client needs to determine relevant services.

Requirements:

  • Tertiary qualification in social work, nursing, or allied health profession.
  • Australian Health Practitioner Regulation Agency (AHPRA) registration or membership of respective professional body.
  • Comprehensive understanding of blood cancer and its impact on clients.
  • Knowledge of supportive care services and resources available to blood cancer clients.
