About the Community Services Liaison Specialist Role

This is a full-time, ongoing position based at Capital Region Community Services Ltd head office in ACT, with the requirement to work from other sites as needed. The role plays an integral part in providing support to individuals and families accessing Commonwealth Home Support Programme (CHSP), Home Care Packages (HCP), Community Assistance and Temporary Supports (CATS) Transport, and National Disability Insurance Scheme (NDIS) Transport services.

The successful candidate will demonstrate a commitment to delivering high-quality advice and support to individuals enquiring or wishing to access services. This important role provides wrap-around support individuals need to gain access to relevant services within CRCS and oversees the process from enquiry to complete onboarding with a warm handover and introduction to the relevant Care Coordinator.

Responsibilities:

  • Understanding assessment outcomes and documentation to assess eligibility for services addressing participants' goals, health, and wellbeing.
  • Providing exceptional customer service and supporting participants to understand information provided, including prices, fees, and charges, agreements, terms related to their rights and responsibilities, and care services.
  • Engaging regularly with participants to support their intake and onboarding process, ensuring they have the necessary information and time to consider their options.
  • Identifying changes in participants' wellbeing, health, mobility, and circumstances, reporting these changes quickly to Care Coordinators for review.
  • Demonstrating a solid understanding of the scope of direct care deliverable under CHSP, HCP, CATS, and NDIS Transport guidelines.

Requirements:

  • Current First Aid and CPR Certificate.
  • Certificate III in aged/community care or equivalent qualification, or extensive work experience.
  • CRCS employees must hold a satisfactory Working with Vulnerable People registration.
  • Employees are required to undertake a National Criminal History Clearance by CRCS prior to commencing and every three years thereafter.
