Job Responsibilities:

  • Support participants through the NDIS journey, assisting with scheme access, plan development, implementation of funded supports and plan reviews.
  • Help participants build confidence, independence and informal support networks.
  • Ensure timely, high quality service delivery in line with performance targets.
  • Build partnerships with local organizations, attend events and promote inclusion through grassroots engagement.
  • Use NDIS and Feros Care systems to document participant interactions, monitor progress, and identify service gaps.

 

Requirements:

  • Experience supporting people with disability in community services, social work, or similar settings.
  • Strong understanding of the NDIS participant pathway and planning processes.
  • Proven ability to manage caseloads and meet service delivery targets.
  • Excellent communication, interpersonal and problem solving skills.
  • Strong community engagement and stakeholder relationship skills.
  • Experience in data entry, reporting and case management systems.
  • Ability to work independently in community based settings.
