Job Responsibilities:
- Support participants through the NDIS journey, assisting with scheme access, plan development, implementation of funded supports and plan reviews.
- Help participants build confidence, independence and informal support networks.
- Ensure timely, high quality service delivery in line with performance targets.
- Build partnerships with local organizations, attend events and promote inclusion through grassroots engagement.
- Use NDIS and Feros Care systems to document participant interactions, monitor progress, and identify service gaps.
Requirements:
- Experience supporting people with disability in community services, social work, or similar settings.
- Strong understanding of the NDIS participant pathway and planning processes.
- Proven ability to manage caseloads and meet service delivery targets.
- Excellent communication, interpersonal and problem solving skills.
- Strong community engagement and stakeholder relationship skills.
- Experience in data entry, reporting and case management systems.
- Ability to work independently in community based settings.
