Support Coordinator Role

This position is the primary point of contact for potential program participants in support coordination, ensuring a comprehensive and timely service package is provided. The role will work closely with NDIS clients to deliver responsive services in line with their support plans.

The ideal candidate will possess strong communication skills, be able to develop and implement effective service plans, and facilitate opportunities for participation, independence, and inclusion in the community.

Key Responsibilities
  • Establish and maintain relationships with NDIS clients and stakeholders to ensure services meet individual needs.
  • Develop and implement service plans that align with NDIS guidelines and client requirements.
  • Identify opportunities for growth and development within support coordination.
Required Skills and Qualifications
  • Desirable - A recognised tertiary degree in Behavioural or Social Science or equivalent.
  • Experience working with families and individuals with disabilities in a support coordination role.
  • Highly developed analytical, evaluation, and planning skills in the disability sector.
  • Excellent written and verbal communication skills.
