Join Calvary Health Care as a Case Manager - Darwin. This permanent role focuses on managing a case load under CHSP/HCP funding, maintaining budgets, onboarding new clients and coordinating care to support client independence.

Responsibilities
Manage a case load of clients under CHSP/HCP approved funding
Maintain budgets and manage care requirements to support client independence
Onboard growth of new clients and coordinate with stakeholders
Time management
Understanding the Aged Care Standards
Liaise with stakeholders and internal networks
What you bring
Minimum 1 year experience in Case Management
Relevant tertiary qualification in the community sector
Experience in aged/community/disability sector, particularly with clients with complex needs
Full Driver's Licence and own reliable registered and insured vehicle
Smart Phone
Knowledge of My Aged Care portal/assessment process
Willing to work in a team environment
A desire to join a team committed to the care of others in a mission-based organisation
NDIS clearance (or willingness to obtain one)
Police clearance (within the last 6 months or willingness to obtain one)
