Job Opportunity: Experienced Senior Social Worker
Job Title: Senior Social WorkerWe are seeking a dedicated and experienced Senior Social Worker to join our Community Geriatrics team in Adelaide's southern suburbs. This challenging and rewarding role offers the opportunity to make a lasting impact on the lives of older adults in our community.
The successful candidate will have the opportunity to work collaboratively as part of a dynamic, inclusive, and supportive work environment, providing comprehensive assessments, tailored care plans, and facilitated access to essential community services.
- Main Responsibilities:
Implementing outcome measures appropriate to the client population to evaluate efficacy of therapy programmes in collaboration with the interdisciplinary team.
Independently assessing client's abilities, problems and needs using a variety of formal and informal assessments as needed.
Providing care planning, case management and liaison to ensure sustainable social support in place in community.
Continually refining and evaluating department protocols, assessment formats, services and processes as part of the auditing/Service Improvement schedule.
About You:
You will have the ability to critically analyse or evaluate issues related to clinical care in social work in a manner which can be applied to quality activities, service evaluation or research.
Experience providing care planning, liaison and case management is highly valued.
Extensive experience in aged care is necessary for this role.
High levels of responsibility, initiative and organisational skills are required.
