Your Key Responsibilities:

  • Conduct detailed aged care assessments to determine the needs of older adults and facilitate referrals to relevant services.
  • Promote the retention and maximization of function and capability in daily living, ensuring that older adults can maintain their independence with dignity.
  • Collaborate closely with families and carers, recognizing them as key stakeholders, and ensure all assessments are conducted in line with Department of Health contractual requirements.

What We're Looking For:

  • A minimum of 1 year of experience in aged care, including client assessments and/or service delivery.
  • Tertiary education in a health-related discipline, such as nursing, occupational therapy, physiotherapy, or social work.
  • Current unrestricted General Registration with AHPRA or relevant professional association, and proficiency in Microsoft Office applications, including Word, Excel, Outlook, and client management systems.
