About the Role

The Senior Team Leader Positive Ageing Care Finders will lead and support the Care Finder team (across a consortium of 3 LGAS) and Support for Carers programs (Darebin only), ensuring delivery of high-quality support. This includes the provision of planned and targeted information and education ensuring clients are sufficiently informed and empowered to make their own choices and engage with appropriate services.

You will develop effective partnerships with Consortium partners Merri-bek and Moonee Valley Council's care finder staff, to achieve the successful implementation of the care finder program across three local government areas.

Key Responsibilities

  • Lead services and resources provided to clients of the Care Finder and Support for Carers programs in accordance with required regulations.
  • Lead and support key assertive outreach activities designed to strengthen identification of Care Finder target population and build relationships with referrers.
  • Manage a high level of mentoring and support to the care finder team including debriefing, regular supervision, case discussion and peer support opportunities.
  • Implement, oversee and evaluate project trials and developments.
  • Provide detailed and complex internal and external reporting in line with funding and service agreements.
  • Contribute to quality improvement programs and activities to work towards maintaining the Quality Management system and Accreditation Standards.
  • Implementing work plans and improvement action plans with the care finder team and conduct reflective practice in all aspects of work to the highest possible standards of service provision.

Requirements

  • Degree in Social Sciences, Allied Health or Community Development.
  • Experience leading teams in Community or Health Services or equivalent combination of relevant experience and/or education training.
