We are seeking a skilled and dedicated Clinical Nurse to join our team at SA Health.
Job SummaryThis is an exciting opportunity for a passionate and experienced nurse to contribute to delivering exceptional healthcare services in Subacute Triage Hub - Division RAP.
Key Responsibilities- Assess referrals to Rehabilitation, GEM and Transition services from acute care, determining client eligibility and providing education to acute care staff.
- Coordinate client services and monitor services provided by external and internal providers.
- Contribute to reporting and quality monitoring mechanisms of the program.
- Registered or eligible for registration as a Nurse by the Nursing and Midwifery Board of Australia and who holds, or who is eligible to hold, a current practicing certificate.
- Effective communication, problem solving, conflict resolution and negotiation skills.
- Ability to work effectively within a multidisciplinary team.
- Ability to prioritise workload and meet set timelines, whilst working under minimal supervision.
- A competitive salary package including $84,870.00 - $107,279.00 (Pro Rata) RN/M2C
