Job Title: Team Leader

At Dhelkaya Health, we are seeking a highly skilled and dedicated Team Leader to join our Social Support Services team. As a key member of our organization, you will play a vital role in leading the planning, development, delivery, and evaluation of programs aimed at promoting physical, emotional, and social well-being.

About the Role

We are looking for a dynamic individual with a passion for social support services and a commitment to promoting independence and well-being. The ideal candidate will have:

  • Proven leadership skills with the ability to inspire and motivate team members.
  • Experience in planning, delivering, and evaluating social support programs, preferably within the healthcare sector.
  • Knowledge of CHSP, HACC-PYP, and NDIS frameworks and funding requirements.
  • Strong interpersonal and communication skills, with the ability to engage effectively with clients, families, carers, and stakeholders.
  • A person-centred care approach, dedicated to meeting the individual needs and interests of clients.
  • Demonstrated ability to maintain compliance with quality standards, policies, and procedures.
  • A collaborative mindset, with a willingness to work closely with colleagues and external partners to achieve organizational goals.
