Aged Care Manager Leader

We are seeking an experienced and skilled Aged Care Manager to lead our team in providing high-quality home care services to clients in Townsville.

As a key member of our leadership team, you will be responsible for:

  • Maintaining exceptional standards of care provided by myHomecare providers and staff.
  • Providing strategic leadership and guidance to the team to achieve our mission of keeping seniors safe at home for longer.
  • Developing, monitoring, and reassessing care plans in consultation with clients, their families, and other healthcare professionals.
  • Collaborating with our Nurses, Regional Managers, and Portfolio Managers to develop and shape our service offerings, ensuring they meet the highest standards.
  • Managing and developing a team, including the Assistant Care Manager and support staff, to deliver exceptional client outcomes.
  • Monitoring budgeted care packages and ensuring clients understand service fees.
  • Overseeing client service delivery across all levels under the government-funded CHSP and Home Care Packages programs.
  • Facilitating client on-boarding through the lead-to-conversion process, working closely with internal teams.
  • Utilizing influencing and partnering skills to lead and work collaboratively with schedulers, clinical teams, and support staff.
  • Ensuring compliance with all relevant legislative and industry standards.
  • Engaging in continuous improvement across practice and service delivery.

To succeed in this role, you will need:

  • Proven recent experience in case management in the Aged Care or Community Services sector.
  • Clinical experience (preferred).
  • Knowledge of Home Care Packages, Commonwealth Home Support Program, and Aged Care Quality Standards is desired.
  • Frontline management skills.
  • A current First Aid Certificate and CPR certification.
  • The right to work in Australia.
  • A current driver's license, registration, and access to a reliable vehicle with comprehensive insurance.
  • National Police Clearance.

This is a challenging and rewarding opportunity for an experienced Aged Care Manager to make a meaningful difference in the lives of our clients. We offer a competitive salary package, including an estimated $95,000 - $110,000 per annum, depending on experience.
