Job Overview:

We are seeking a skilled professional to fill the role of Rehabilitation Coordinator. This position requires strong practical experience in identifying, reviewing, and coordinating services for individuals experiencing serious illness or injuries or disability.

Key Responsibilities:

  • Coordinate person-centered, timely, high-quality, and cost-effective services to meet participants'/workers' needs and scheme objectives.
  • Use a person-centered approach to assist in identifying and reviewing the needs of participants/workers across all phases of recovery and community participation.
  • Facilitate support to enhance each participants'/workers' capacity to achieve their desired goals, as identified in their My Plan.
  • Work collaboratively with internal specialist services, external case managers, health professionals, and other key stakeholders to coordinate the most effective options to meet the treatment, care, and support needs of the participant/worker.
  • Manage service approvals and payments in accordance with agreed needs, assessments, and plans.
  • Adhere to KPIs that monitor and measure key areas of the role.
  • Keep abreast of and understand the legislative framework and Guidelines and how these contribute and impact the decision-making framework.
  • Work collaboratively with all teams across Care, in the design and implementation of internal quality assurance, management of disputes and complaints, and continuous improvement systems to drive quality and consistency in decision making.

Requirements:

  • A sound understanding of the needs and perspectives of people with disabilities.
  • Critical reasoning skills to ensure a high degree of integrity in the decision-making processes.
  • Excellent communication skills - ability to build rapport and communicate complex information and create and nurture effective working relationships with all stakeholders.
  • Ability to set clear expectations and engage in difficult conversations in a simple and empathetic manner.
  • Ability to travel within NSW and possibly interstate depending on the Region.
  • A current driver's licence.
  • Compliance with Pre-employment checks (including Working With Children Check).
  • Up to date with current Public Health Order requirements.
