Healthcare Professionals: Aged Care Assessor Role

As we continue to expand our operations, we have a 6 month Fixed Term Clinical Assessor role available on the Central Coast.

Key Responsibilities
  • Conduct comprehensive assessments to identify and meet the complex needs of clients, referring them to appropriate aged care services as required.
  • Collaborate with clients, families, and multidisciplinary teams to ensure seamless delivery of care.
  • Work collaboratively with colleagues to foster a supportive and positive team culture.
  • Conduct thorough aged care assessments using the Integrated Assessment Tool (IAT) to determine client needs and eligibility for home support services.
  • Review and adjust support plans to meet changing client needs.
Requirements
  • Hold a tertiary qualification in a health-related discipline directly related to health, aged care, or a related specialist area.
  • Be currently registered with the Australian Health Practitioners Regulation Agency (AHPRA).
  • Possess an Australian Driver's license and access to a registered, insured vehicle.
  • Demonstrate at least one year of experience delivering services in aged care settings and/or to aged persons, including those living with dementia.
