Key Role in Aged Care

Responsibilities
  • Conduct accurate assessments using the Integrated Assessment Tool (IAT) to ensure optimal care for clients.
  • Collaborate with multidisciplinary teams to promote a culture of collaboration and shared goals.
  • Develop and implement individualized support plans to meet the unique needs of each client.

Effective communication and interpersonal skills are essential in this role, as you will work closely with clients, families, and healthcare professionals to deliver high-quality care.

Requirements
  • Tertiary qualification in a health-related field directly related to Health, Aged Care, or a specialist area such as Nursing, Occupational Therapy, or Social Work.
  • Current registration with the Australian Health Practitioners Regulation Agency (AHPRA).
  • Australian Driver's license and access to a registered vehicle.
  • Minimum one year experience delivering services in Aged Care settings, including those living with dementia.
  • Ability to maintain confidentiality and handle sensitive information with discretion.
