Job Opportunity:

Clinical Assessor

We are seeking a highly skilled and experienced Clinical Assessor to join our team. The successful candidate will be responsible for conducting assessments to identify and meet the complex needs of clients, referring them to appropriate aged care services under the Aged Care Act.

  • Main Responsibilities:
  • Collaborate with clients, families, and multidisciplinary teams as required.
  • Conduct accurate aged care assessments using the Integrated Assessment Tool (IAT) to determine client needs and eligibility for home support services.
  • Review support plans to meet client's changing support needs.
  • Work effectively both autonomously and as a member of a broader team in delivering to key performance indicators within identified timeframes.
  • Maintain currency of knowledge relating to aged care services both funded and non-funded.
  • Assist with continuous improvement frameworks to regularly evaluate and enhance assessment processes and outcomes.

Requirements:

  • Tertiary Qualification - in a health-related discipline directly related to Health, Aged Care or related specialist area (e.g. Registered Nurse, Medical Officer, Occupational Therapist, Physiotherapist or Social Worker).
  • Current unrestricted registration with the Australian Health Practitioners Regulation Agency (AHPRA).
  • An Australian Driver's license and access to a registered, insured vehicle.
  • Minimum of one year demonstrated experience in Australia or Overseas directly delivering services in Aged Care settings and/or to Aged persons including to people living with dementia.
