About the Role

We are seeking a highly skilled and compassionate Care Manager to join our team at Myhomecare. As a Care Manager, you will play a vital role in delivering exceptional telephone-based client care to elderly Australians.

Key Responsibilities
  • Develop and implement personalized care plans in consultation with clients, their families, and healthcare professionals.
  • Deliver high-quality care coordination, assessment, and monitoring services to ensure clients receive the best possible support.
  • Collaborate with our nursing team, regional managers, and other stakeholders to develop and refine our service offerings.
  • Manage client service delivery across all levels of home care packages, ensuring seamless and efficient support.
  • Engage with internal teams to facilitate client onboarding and ensure compliance with relevant legislative and industry standards.

Requirements
  • Excellent communication and interpersonal skills.
  • Experience in aged care, with knowledge of Home Care Packages, Commonwealth Home Support Program, and Aged Care Quality Standards.
  • Outstanding computer skills in CRM systems, database management, and Microsoft Office.
  • The right to work in Australia.
  • National Police Clearance or willingness to obtain.
