In this pivotal role, you will oversee the accounts of vulnerable clients and collaborate with healthcare professionals to ensure seamless care transitions.
With a deep understanding of funding structures and alternative care options, you will work proactively to safeguard clients' interests and continuity of care. Your expertise will be invaluable in guiding case management decisions, fostering trust, and delivering effective services.
- Evaluate individual situations of escalated cases, ensuring case management is handled with dignity and respect.
- Develop transparent, compassionate guidelines for funding-based decisions that meet regulatory standards.
- Communicate with clients and families regarding account status, care options, and funding decisions while promoting understanding and trust.
- Collaborate with interdisciplinary teams to explore alternative support solutions for clients at risk of service suspension.
- Maintain detailed documentation of actions taken during the suspension process.
- Provide staff training on best practices for managing delicate client interactions and safeguarding our organisation's reputation.
- Stay informed about NDIS and HCP policy changes and incorporate new regulations into client care strategies.
- Advocate for clients' needs and rights, upholding our organisation's ethical standards.
Requirements:
- Bachelor's degree in Finance, Social Work, Healthcare Administration, or related field.
- Proven experience in case management, specifically within NDIS and HCP settings.
- Deep understanding of NDIS and HCP funding mechanisms and regulatory requirements.
- Strong crisis management skills and ability to handle sensitive issues discreetly.
- Excellent communication and interpersonal skills, with negotiation and advocacy capabilities.
- Highly developed organisational and time management skills.
- Expert problem-solving abilities with a focus on ethical decision-making.
