Journey Support Specialist Role:

We are seeking a skilled individual to fill the role of Journey Support Specialist within our Stepped Care service system. As a Journey Support Specialist, you will play a crucial role in ensuring that individuals are supported through their care journey.

Key Responsibilities:

  • Establish and maintain relationships with individuals, teams, and community members
  • Deliver high-quality support and services to improve outcomes for Stepped Care participants
  • Facilitate the Stepped Care model to ensure quality, continuity, and coordination of care between service providers

Required Skills and Qualifications:

  • Ability to establish, develop and maintain multiple productive working relationships and networks
  • Experience identifying and delivering partnership and collaboration opportunities that enhance consumer outcomes and improve the service system
  • An understanding of mental health challenges and how they inform an individual's experience
  • Minimum Certificate IV in any Mental Health related qualifications
