We are seeking a full-time Comprehensive Care Specialist to deliver exceptional care to patients with varying complexity on a fixed-term contract. - Collaborate with a multidisciplinary team to develop and implement integrated service models and clinical protocols.

- Support seamless transitions and facilitate access to necessary resources.

Key Requirements:
