Disability Support Coordinator

We are seeking a highly skilled and dedicated professional to fill the role of Local Area Coordinator. This position plays a crucial role in supporting individuals with disabilities, their families and carers.
The successful candidate will work closely with the NDIS to help people build and pursue their goals, exercise control and engage with the system.
Your Key Responsibilities- Plan Development: Assist NDIS-eligible individuals and their support networks in developing personalized plans that align with their goals and aspirations.
- Awareness and Outreach: Work collaboratively with community-based organizations to raise awareness about the NDIS and its benefits.
- Progress Monitoring: Monitor and evaluate plan progress, providing ongoing support to ensure successful activation and implementation.
- A current Working with Children Check (Employee)
- NDIS Workers Screening Check (can be obtained during application)
- Qualifications in or lived experience of disability will be highly regarded (desirable)
