
Care Team Leader

The Clinical Coordinator position is pivotal in providing expert guidance and direction to care teams.
Key Responsibilities:
- Provide professional leadership and supervision of care teams, encompassing Nurses, Personal Carers & Allied Health staff.
- Develop comprehensive care plans in collaboration with residents, family members/caregivers, and other care team members.
- Promote a service delivery culture that focuses on enhancing the lifestyle experiences of residents.
- Maintain accurate and up-to-date documentation systems.
Required Qualifications and Experience:
- Current Division 1 Registered Nurse registration with AHPRA.
- Minimum 2+ years' experience as a Registered Nurse.
- Demonstrated expertise in clinical governance, accreditation, and AN-ACC.
- Valid working rights in Australia (Passport, Birth Certificate etc).
- Background checks and references.
- Current flu season vaccination (or willingness to obtain).
- Evidence of Covid-19 vaccination.
- NDIS Clearance (or willingness to obtain prior to commencement).
- Qualifications / AHPRA.
- Complete a police check.
