About the Opportunity
Our organisation is seeking a dedicated and experienced Senior Aged Care Professional to join our Clinical Leadership team. As a key member of this team, you will play a pivotal role in shaping clinical governance activities, including planning and implementing clinical priorities.
About the Role
- Develop and implement a comprehensive care plan review program to ensure that care plans and assessments remain current and relevant.
- Contribute to the development of training and education plans to support employees in completing assessments, care plans and documentation.
- Regularly analyse psychotropic, antipsychotic, polypharmacy and antimicrobial stewardship, and report and consent obligations.
- Assess and plan for new admissions and existing residents in consultation with internal and external stakeholders.
- Monitor the delivery of care to ensure consistency with the care plan and goals, taking action if gaps or inconsistencies are identified.
- Review care incidents and care change patterns for residents, collaborating with the Clinical Care Team to determine if further assessment and care planning modifications are needed.
Key Responsibilities
- Establish relationships with internal and external stakeholders to facilitate effective communication and collaboration.
- Work collaboratively with the Clinical Care Team to develop and implement evidence-based practices and policies.
- Identify areas for improvement and contribute to the development of strategies to enhance care quality and outcomes.
- Participate in regular meetings and discussions to stay informed about organisational initiatives and objectives.
About You
- Registered Nurse with at least 2 years' clinical experience.
- Aged care management experience and familiarity with the Quality Standards for Aged Care.
- Excellent verbal, written and interpersonal communication skills.
- Knowledge and ability to use computers and technology efficiently.
- Current Police Check and appropriate Working Rights.
- Up-to-date vaccinations (Influenza, Covid).
- Experience in a similar position in Aged Care - desirable.
- Management qualifications - highly regarded.
