About the Role

We are seeking a dedicated and compassionate Registered Nurse to join our team as a Care Coordinator. As a key member of our team, you will play a vital role in delivering high-quality care and support to our clients.

Key Responsibilities
  • Build strong relationships with clients and their families, understanding their unique needs and preferences.
  • Assign staff to clients based on their qualifications, availability, and client preferences.
  • Organize support for clients with disabilities, monitoring and reporting on their progress.
  • Refer clients to agencies that can provide additional help, assessing their needs and resources.
  • Respond promptly to client inquiries, maintaining accurate records of incidents and feedback.
  • Reorganize work priorities to meet the demands of a fast-paced environment, supporting clients to meet their goals.
  • Conduct Client Care Assessments and Care Planning, providing clinical guidance to team members.
  • Discuss and promote Right at Home's services with medical professionals and referral sources.
Requirements
  • Current AHPRA registration as a Registered Nurse.
  • Experience in assessing and planning care needs.
  • Understanding of the Aged Care and Disability systems.
  • Strong client focus, based on their goals, preferences, and needs.
  • Current National Police Check, NSW Working with Children Check, and NDIS Worker Screening Check.
  • NSW Full Driver's Licence.
  • All candidates must have the right to work in Australia.
