About the Role:

We are seeking a dedicated and compassionate Registered Nurse to join our team as a Care Coordinator. As a Care Coordinator, you will play a vital role in building strong connections with our clients and their families, ensuring that they receive the highest quality care and support.

Key Responsibilities:

  • Building strong relationships with clients and their families to understand their needs and preferences
  • Assigning staff to clients based on their qualifications and availability
  • Organizing support for clients with disabilities and monitoring their progress
  • Referring clients to agencies that can provide additional help
  • Assessing client needs and resources for health, welfare, and other facilities and services
  • Responding promptly to client requests and maintaining accurate records of incidents and feedback
  • Re-organizing work priorities to meet the needs of a fast-paced environment
  • Supporting clients to meet their goals and conducting Client Care Assessments and Care Planning
  • Providing clinical guidance to other team members for client management
  • Discussing and promoting Right at Home's services with medical professionals and referral sources

Requirements:

  • Current AHPRA registration as a Registered Nurse
  • Experience in assessing and planning care needs
  • Understanding of the Aged Care and Disability systems
  • Strong client focus based on their goals, preferences, and needs
  • Current National Police Check (or commitment to obtain prior to start)
  • NSW Working with Children Check (or commitment to obtain prior to start)
  • NDIS Worker Screening Check (or commitment to obtain prior to start)
  • NSW Full Drivers Licence
  • All candidates must have the right to work in Australia
