We are seeking a skilled Care Coordinator to join our team.

Key Responsibilities- To develop, implement, and review care plans that promote independence and quality of life.
- To lead and supervise team members, providing supervision, guidance, and mentorship.
- To respond to clinical nursing issues and concerns, ensuring the delivery of high-quality care.
This role offers the opportunity to work with a large team to coordinate care and support the needs of residents.
You will be responsible for developing and implementing care plans, monitoring their effectiveness, and making adjustments as necessary.
Requirements- Bachelor of Nursing or equivalent qualification.
- Current AHPRA registration.
- Experience coordinating care within a large team.
- Ability to manage conflict, pressures, and competing priorities.
