About the RoleThis dynamic role oversees complex client care plans, ensures Community Support Professionals are trained and assessed in advanced clinical skills, and maintains the highest standards of care, compliance and professionalism.
Key Responsibilities:- Lead continuous quality improvement initiatives to ensure compliant services.
- Oversee medication administration and clinical interventions for clients with complex needs.
- Deliver training, coaching and skills assessments for Home Care Workers.
- Manage incident processes and ensure timely resolution.
- Support customers living with dementia or cognitive impairment through comprehensive assessments and personalized strategies.
- Conduct falls risk assessments and implement prevention strategies to promote client safety.
- Work in close proximity to home, assisting people in your local community.
- Hybrid working conditions available once training is completed.
