Job Description

The Family Wellbeing Practitioner plays a pivotal role in empowering families to overcome identified challenges and achieve effective change. In partnership with the family, this practitioner facilitates a client-led assessment and care plan, identifying goals for the entire family where appropriate.

This dynamic role involves providing high-quality, intent-based practice consistent with the organisation's values and vision. The practitioner will facilitate targeted interventions and family facilitated group meetings to help clients achieve their goals.

  • Meet individual case performance targets as per weekly, monthly, and quarterly dashboard system.
  • Provide high-quality and intent-based practice in all interactions with clients, consistent with the organisation's values and vision.
Requirements

To be successful in this role, you will require:

  • A current Bachelor's degree in Social Work, Human Services, or related field, accompanied by 2 years' relevant experience in social services, or;
  • A current Diploma in Community Services, Social Work, or related field, accompanied by 3 years' experience in social services, and at least 1 year of abovementioned experience in a case management role.

  • Demonstrated ability and comfort working in a performance-driven environment to meet individual weekly, monthly, and quarterly performance targets.
  • Demonstrated ability to build client agency, self-efficacy, and facilitate change empowerment while discouraging client dependence on supports.
  • Demonstrated knowledge and ability to provide behaviour change interventions that build client self-awareness and active change in achieving their identified goals.
