About the Role

A Home Care Support Specialist is responsible for providing personalized care and support to customers in their own homes. This role involves assisting customers with daily living activities, such as personal care, medication management, and household tasks.

We are seeking a kind-hearted and empathetic individual to join our team as a Home Care Support Specialist. The successful candidate will have a Certificate III or IV in Individual Support (Ageing) or equivalent, and a Medication Competency Certificate. They will also have strong interpersonal and communication skills, and be able to work independently and as part of a team.

Key Responsibilities

  • Provide personalized care and support to customers in their own homes
  • Assist customers with daily living activities, such as personal care, medication management, and household tasks
  • Develop and implement care plans that promote independence and well-being for customers
  • Work collaboratively with other healthcare professionals to ensure seamless care and support
  • Maintain accurate records and reports of customer interactions and care provided

Requirements

  • Relevant Certificate III or IV in Individual Support (Ageing) or equivalent
  • Medication Competency Certificate
  • Current First Aid Certificate and CPR
  • Strong interpersonal and communication skills
  • Ability to work independently and as part of a team
  • Driver's license and a fully insured car
