About this Opportunity

Wellways Australia Limited is seeking a dedicated and compassionate Home Care Worker to join our team. As a Home Care Worker, you will play a vital role in supporting our participants with their daily activities, promoting their independence, and enhancing their overall quality of life.

Key Responsibilities

  • Deliver high-quality care and support to participants in their homes, promoting their physical and emotional well-being.
  • Develop and implement personalized care plans that cater to the unique needs and goals of each participant.
  • Work collaboratively with participants, their families, and other healthcare professionals to ensure seamless care coordination.
  • Provide companionship, meal preparation, domestic assistance, and medication management, as required.
  • Contribute to the continuous improvement of care services, identifying opportunities for growth and development.

Requirements

  • Certificate III in Mental Health, Disability, Aged Care, or Home and Community Care (desirable but not essential) or equivalent working or lived experience.
  • A current valid Driver's License and a registered, insured vehicle.
  • Police Records Check (completed within the last 12 months).
  • Working with Children Check.
  • Manual handling training is desirable, but you will receive training if needed.
  • Rights to work in Australia.
  • Availability and willingness to use personal smartphone.
  • NDIS Workers Screening Check.
  • NDIS Worker Orientation Module Certificate – free online course.
