About the Role

Key Responsibilities:
  • Collaborate with families, caregivers, and clinical staff to provide holistic support for individuals living with dementia.

  • Conduct assessments to gain a deeper understanding of each individual's experience and develop targeted reports that drive positive change.
What We Need from You:
  • Experience working with individuals living with dementia and a strong understanding of person-centred care approaches.
  • Relevant qualifications in healthcare, nursing or social work are essential.
  • Excellent communication, relationship building and problem-solving skills.
