Key Responsibilities

  • Conduct comprehensive assessments to identify client needs and develop effective treatment plans
  • Work collaboratively with other healthcare professionals, including GPs, Allied Health Professionals, and Case Managers, to ensure seamless care delivery
  • Engage with clients and their families to understand their goals and preferences, and develop personalized care plans
  • Provide education and support to clients and their families to promote independence and self-management
  • Participate in multidisciplinary team meetings to discuss client progress and develop care plans
