Senior Occupational Therapist

This is an exciting opportunity to join our dynamic team as a Senior Occupational Therapist in the Older Persons Emergency Referral and Assessment (OPERA) service. As a key member of our multidisciplinary team, you will play a vital role in supporting older individuals during their transition from hospital to home or residential care.
Key Responsibilities- Conduct comprehensive assessments to identify the needs of older adults and develop tailored intervention plans to promote functional independence and safety.
- Work collaboratively with a multidisciplinary team to ensure seamless care coordination and optimize health outcomes.
- Develop and maintain strong relationships with clients, families, and healthcare providers to ensure person-centered care.
- Contribute to the professional development of junior staff and foster a culture of continuous learning and excellence within the team.
