Job Title: Intake Coordinator

This role is responsible for managing the initial assessment and registration process for a variety of healthcare services across Primary Care and Community Health.

Key Responsibilities:
  • Receive referrals from various sources, providing a warm and empathetic response while navigating service eligibility criteria.
  • Navigate referral systems, provide an empathetic response, and offer referral options to vulnerable individuals.
  • Apply clinical judgment and triage skills in situations involving suicidal or distressed individuals, domestic violence, and child protection issues.
Requirements:
  • A minimum of a Certificate IV or Diploma in Welfare Work (or similar).
  • Demonstrated recent experience working in a welfare role.
  • Experience using IT systems and client registration systems.
