Personalised Support Worker

We are seeking a skilled and compassionate Personalised Support Worker to deliver tailored support to participants in their own homes and community.

  • Deliver personalised support services, encompassing daily living activities such as grooming, toileting, meal preparation, medication administration, and household management.
  • Provide companionship and emotional support, respecting client preferences and privacy.
  • Collaborate with clients and their families to develop and implement individualised support plans.
  • Communicate effectively with clients, healthcare professionals, and other stakeholders to ensure seamless care delivery.
Requirements

To be successful in this role, you will possess:

  • A minimum of 6 months' experience or a Certificate III/IV in Aged Care, Home & Community, Disability, or a related field.
  • Proven experience working in in-home or community care settings, preferably within the disability sector.
  • Excellent communication and interpersonal skills, with the ability to establish professional boundaries.
  • A valid NDIS Worker Screening Check, First Aid Certificate, Driver's Licence, and access to an insured vehicle.
