Job Title: Care Manager
The primary responsibility of a Care Manager is to oversee the effective delivery of care and clinical services to residents. This involves leading the care team, working closely with other departments, and ensuring resident-centric care.
Key Responsibilities:
- Develop comprehensive care plans based on individualized assessments
- Monitor medication management and ensure accurate documentation
- Manage comments and complaints in accordance with organisational requirements
About You
To succeed in this role, you will need strong communication skills, leadership qualities, and a commitment to resident wellbeing. Key requirements include:
- Minimum 12 months experience in a leadership position or similar role
- Bachelor of Nursing degree or equivalent qualification
- Valid registration with relevant professional body
- Experience in preparing and conducting clinical assessments
- Familiarity with relevant compliance requirements
