Case Manager

We are seeking a dedicated and empathetic professional to join our team as a Case Manager in our Limestone Coast Domestic Violence Service. As a key member of our team, you will play a vital role in providing tailored case management and high-quality, client-focused services to domestic violence victim-survivors and their children.
Key Responsibilities- Conduct risk assessments using high-quality domestic and family violence practices and trauma-informed approaches.
- Perform Home Safety Audits to identify vulnerabilities and implement risk-mitigating strategies.
- Coordinate property security upgrades to enhance safety.
- Build and maintain relationships with contractors across regions, adhering to Centacare's Contractor Management policies.
- Facilitate Safety Planning in collaboration with clients to ensure their safety.
- Make timely referrals to additional support services as needed.
You will possess a strong background in domestic and family violence practices, with excellent skills in conducting risk assessments and home safety audits. Your ability to identify vulnerabilities and implement effective security measures will be key to your success. A commitment to trauma-informed practices and a passion for enhancing the safety and wellbeing of women and children are crucial traits for this role.
Requirements- Degree in Social Work and/or demonstrated experience commensurate with the position.
- Ability to travel interstate when required.
- Ability to undertake some work out of standard work hours when required.
- Valid DHS Working with Children check (WWCC).
- Valid Drivers Licence.
- SCHCADS Award 2010 Crisis Accommodation Level 3 $49.28 per hour + super.
