Job Title: Community Support Specialist

The role of a Community Support Specialist is an integral part of Centrecare's operations, focusing on delivering comprehensive support services to individuals in need.


Key Responsibilities:
  • Conduct thorough assessments to develop personalized support plans and referrals based on identified needs.
  • Implement evidence-based case management and counseling techniques to facilitate offenders' successful reintegration into their communities.
  • Maintain accurate documentation, including case notes, assessments, and support plans.
  • Foster collaborative relationships with internal and external stakeholders, ensuring seamless communication and cooperation.
  • Provide critical risk assessment and safety planning for individuals at risk of family and domestic violence.
  • Support individuals in crisis through targeted specialist referrals and tailored support programs.

Requirements:
  • Bachelor's degree in Psychology, Criminology, Social Work, or a related field.
  • Diploma in Community Services, Counseling, or a similar field (Level 4 position).
  • Minimum 1-2 years of experience working in a similar position (Level 4 position).
  • Minimum 3 years of experience working in a similar position (Level 5 position).
  • Clinical and counseling experience in adult justice reintegration support settings.
  • Proven experience working within the justice sector.
  • Ability to work effectively with Aboriginal and Torres Strait Islander peoples and communities in a culturally responsive manner.
  • Capacity to obtain a National Criminal History Record Check and Integrity Check (Justice Screening Clearance).
