We are seeking a dedicated and experienced Clinical Care Coordinator to join our team. This full-time role will involve providing clinical expertise and leadership to our care teams, ensuring that our residents receive the highest quality of care.

Job Description

This is an exciting opportunity for a skilled and compassionate individual to make a real difference in the lives of our residents. As a Clinical Care Coordinator, you will be responsible for:

  • Providing clinical expertise and leadership to our care teams
  • Assisting the Care Manager to lead carer teams to meet all clinical needs of residents
  • Managing all aspects of assessment, documentation, and submission of AN-ACC requirements within the facility
  • Supporting the effective documentation and full clinical team user support of policies, procedures, and IT system requirements in the facility
  • Coordinating assessment, care planning, and review processes within the facility
Requirements

To be successful in this role, you will need:

  • A minimum 1 year experience as a Care Coordinator or similar leadership position
  • A current unrestricted Registered Nurse Div 1 with Australian Health Practitioners Regulation Agency (AHPRA)
  • Post graduate qualifications in aged care would be advantageous but not essential
  • Experience and extensive knowledge of Aged Care clinical assessments, care plans, AN-ACC, and accreditation compliance and documentation
  • An engaging style that has ability to easily influence and develop clinical leadership skills across the clinical care team in an aged care environment
  • A high level of computing skills to ensure that documentation and reporting requirements can be maintained in accordance with organisational and legislative requirements
