Opportunity Overview

We are seeking a dedicated Clinical Care Coordinator and Nurse Mentor to join our team.

The Role

This full-time position involves leading clinical and care service teams across the facility, providing clinical expertise, and mentoring staff to achieve best practice clinical care outcomes. The successful candidate will be responsible for managing all aspects of assessment, documentation, and submission of AN-ACC requirements within the facility, ensuring accurate information is recorded and in accordance with organisational and legislative requirements.

  • Leadership and management of clinical and care service teams across the facility
  • Assist the Care Manager to lead carer teams to meet all clinical needs of residents
  • Responsible for managing all aspects of the assessment, documentation, and submission of AN-ACC requirements within the facility
  • Support the effective documentation and clinical team user support of policies, procedures, and IT system requirements in the facility
  • Coordination of assessment, care planning, and review processes within the facility

Key Qualifications and Requirements:

  • Current unrestricted Registered Nurse Div 1 with Australian Health Practitioners Regulation Agency (AHPRA)
  • Minimum 1 year experience as a Care Coordinator or similar leadership position
  • Post graduate qualifications in aged care would be advantageous but not essential
  • Experience and extensive knowledge of Aged Care clinical assessments, care plans, AN-ACC, and accreditation compliance and documentation
  • Engaging style that has ability to easily influence and develop clinical leadership skills across the clinical care team in an aged care environment
  • High level of computing skills to ensure that documentation and reporting requirements can be maintained in accordance with organisational and legislative requirements
  • Confident team player with excellent communication and interpersonal skills
  • Ability to manage multiple stakeholders and have a balanced and level-headed approach to emergency and stressful situations
