Case Manager

We are seeking a skilled and dedicated individual to provide case management and support to vulnerable children, young people, and their families who are being impacted by disadvantage.

About the Role:

  • Provide case management and support to individuals and families experiencing poverty, trauma, substance abuse, mental health issues, and family violence.
  • Support clients who are potentially at risk of becoming involved with the Child Protection system or are already involved with child protection.
  • Develop and implement case plans to address the needs of clients and improve their well-being.
  • Collaborate with other professionals and services to ensure comprehensive support for clients.

Key Responsibilities:

  • Provide intensive case management and support to high-risk clients.
  • Develop and implement case plans that address the complex needs of clients.
  • Collaborate with internal and external stakeholders to ensure seamless service delivery.
  • Conduct regular home visits and provide support to clients in their homes.

Requirements:

  • Tertiary qualification in human services, disability, welfare, or social work.
  • Current eligible motor vehicle license to drive in Australia.
  • Sound knowledge of the underlying principles of the Children, Youth and Families Act 2005.
  • Demonstrated experience working with young people and families.
  • Excellent knowledge of case management and issues that impact children, youth, and their families.
