Job Overview:

  • Provide support services in accordance with client plans.
  • Assist clients with daily living activities such as personal care, transportation, and respite care.
  • Keep accurate records of client progress and care plans.
  • Communicate effectively with clients, families, and other care team members.
  • Attend meetings to review and adjust client plans as needed.
  • Offer training and education on navigating support systems.

Key Responsibilities:

  • Support clients with high levels of need through ongoing personal care and assistance.
  • Provide regular communication updates to clients, families, and carers.
  • Develop strong working relationships with multidisciplinary teams.
  • Implement a person-centered approach to supporting diverse needs.
  • Identify and address client needs through proactive planning.
  • Promote active ageing and participation in community life.
  • Manage risks associated with supporting people with complex needs.
  • Manage challenging behavior using de-escalation techniques.
  • Maintain confidentiality and professional boundaries at all times.
  • Comply with organisational policies and procedures.

Requirements:

  • High school qualification or equivalent.
  • At least 1 year experience working with people with disabilities.
  • Strong understanding of relevant legislation and regulations.
  • Excellent communication and interpersonal skills.
  • Ability to work collaboratively within a multidisciplinary team environment.
  • Knowledge of CRM software and support portal applications.
  • First aid qualifications.
  • Ability to work with a diverse client base and understand individual needs.
  • Basic computer skills and proficiency in Microsoft Office Suite.
